Badge Information For Employees
Staff at Remote Locations
Please
do not
reach out directly to non-NIH HHS badge offices for badge assistance. AOs should contact
ORS Personnel Security
regarding remote badge services. DPSAC will coordinate the appointments at remote locations.
When badge services are needed, Administrative Officers (AOs) should complete the
Request for Remote Badging Services Form
" provided on the ORS site below and submit it to DPSAC so that the coordination/approval process can be initiated.

Individuals with No Social Security Number
DPSAC is not able to issue an HHS ID Badge to individuals (foreign visitors, volunteers, etc.) that do not have and will not have a Social Security Number. If an ID badge is required for accessing the NIH, these individuals must obtain an Extended Visitor badge from the NIH Police.
Non-NIH Individuals
Extended Visitor Badge
These badges are issued to individuals not affiliated with NIH but need frequent physical access to NIH facilities. The security process requires the sponsorship form to contain the AO signature and justification and requires applicants to have their fingerprints electronically captured and submitted through the Office of Personnel Management (OPM) for criminal history screening.

Authorization for Release of Information Form (Extended Visitor ID Badge Application):
https://security.nih.gov/Documents/Extended%20Visitor%20Form.pdf#search=documents
Cost
The cost of sponsorship of non-NIH personnel to the sponsoring IC is $21.50 per person. A justification statement by the sponsor as to why the individual requires employee-like access to the NIH campus is required.
Extended Visitor Fingerprinting and Badge Issuance Locations
NIH Gateway Center (next to Metro Plaza, South Drive entrance)
Building 10, 1C52 (Clinical Center South Lobby)
Patients
The sponsorship of patient badge applications is limited to the patient's care/research team. The new sponsorship forms are available in CRIS. Patients are not required to be fingerprinted so there is no charge to the IC.
Patient Family Members or Caregivers
Issuance of badges to family members or patient caregivers will be limited to two (2) per patient and the sponsorship of their badge applications will be limited to hospitality staff at the P1 Lobby Hospitality Desk. Fingerprinting is required and there will be a charge of $21.50 per person to the IC that sponsored the patient badge.

Badge Assistance
Lost or Stolen Badge
If you have lost your HHS ID Badge, or believe it has been stolen, contact your Administrative Officer (AO). Your AO can report the badge as lost or stolen in NED, which submits a new badge request.
Note:
NED does not allow AOs to cancel ID badge reissuance processes after reporting badges as lost or stolen. Once an AO or AT reports an ID badge as lost or stolen in NED, the badge holder must obtain a new badge even if they find their old badge.
Broken Badge
To protect your HHS ID Badge, be sure to replace it in its protective badge holder after every use. If your badge is damaged, you need to take your broken badge the badge issuance station in Building 31, Room 1B03 for replacement.
Badge Renewal
Please see the following website for information on renewing an existing HHS ID Badge:
